Phoebe's Geriatric Care Manager will evaluate the needs of the client through a comprehensive assessment process and establish a plan of care based on the client's strengths, personal values, interests, current environment and resources. The GCM will identify and coordinate community services to best meet the needs of the client.

Families living out of town or leading busy lives can depend upon Phoebe's Geriatric Care Manager to assist with support and the coordination of services for the client. The GCM has the expertise to help the client and family understand what is needed to enable the client to remain in the comfort and safety of their own home.
